Measurement Form

Fill in ALL the information below. Have a Formalwear store or Men's store take your measurements. DO NO TAKE THEM YOURSELF! Please give one weeks notice. A $20 deposit must accompany each order and will be forfeited if cancelled. Send check, money order or credit card number and date for pick-up. The $20.00 goes toward your rental, and the balance must be paid when order is picked up.
